Data from the Phase III comparative clinical trial demonstrated that the investigational biosimilar candidate, CT-P47, has an equivalent efficacy, and comparable safety and immunogenicity profile to the reference tocilizumab product.The Phase III study randomised 471 patients with moderate to severe RA to receive either CT-P47 or reference tocilizumab every 4 weeks at a dose of 8 mg/kg for the first 20 weeks. Prior to dosing at week 24, patients receiving tocilizumab were re-randomised either to continue with reference tocilizumab or undergo transition to CT-P47 up to week 48. The primary endpoint was disease activity score 28 (DAS28) erythrocyte segmentation rate (ESR), improvement at weeks 12 and 24. The estimated differences between the two groups were -0.01 at week 12 and -0.1 at week 24, and the confidence intervals of the difference were entirely within the pre-defined equivalence margins at both timepoints (95% CI: -0.26 to 0.24 at week 12 and 90% CI: -0.30 to 0.10 at week 24). The treatment groups were highly similar in terms of mean serum concentration up to week 32, incidence of treatment emergent adverse events and anti-drug antibody positivity, demonstrating comparable pharmacokinetics, safety and immunogenicity. “Biosimilars provide an opportunity to address unmet medical needs by expanding access to high-quality biologic medicines. The positive top-line results from the Phase III study supports the biosimilarity of CT-P47 to reference tocilizumab, and also provide clinical evidence for the possibility of switching from reference tocilizumab to CT-P47,” said Dr. Josef S. Smolen, Emeritus Professor of Medicine at the Medical University of Vienna, Austria. In addition, a Phase I/III study compared pharmacokinetic (PK) similarity between the CT-P47 autoinjector (AI) and the pre-filled syringe (PFS) and evaluated the usability of the CT-P47 AI.2 The study showed PK similarity of the CT-P47 AI and the PFS in healthy subjects. In addition, higher usability of the CT-P47 AI was observed compared to the PFS in RA patients. Celltrion also presented endoscopic outcomes from the post-hoc analysis of the LIBERTY-CD study.3These data were presented as poster presentations at the 19th European Crohn’s and Colitis Organisation (ECCO) annual congress in Stockholm, Sweden. Two-year results of CT-P13 SC LIBERTY studiesThe LIBERTY-CD and LIBERTY-UC studies were continued up to 102 weeks as extension phase treatments, building on the initial LIBERTY trials. The two-year studies assessed the long-term efficacy and safety of CT-P13 SC in patients with Crohn's disease (CD) and ulcerative colitis (UC), respectively.In the CT-P13 SC LIBERTY-CD study, a total of 180 patients with moderately to severely active CD entered into the extension phase up to Week 102 and received CT-P13 SC 120mg regardless of the previous assigned arm of maintenance phase. 154 (85.6%) patients completed the extension phase, and efficacy results, including clinical remission, clinical response, endoscopic remission, endoscopic response, and corticosteroid-free remission, were generally maintained at Week 102 compared to Week 54. No new safety issues were reported during the extension phase.In the CT-P13 SC LIBERTY-UC study, a total of 237 patients with moderately to severely active UC entered into the extension phase and received CT-P13 SC regardless of the previous treatment group randomized at the start of maintenance phase. The LIBERTY-UC study demonstrated sustained efficacy through Week 102, with 208 (87%) patients completing the extension phase. Clinical remission, clinical response, endoscopic-histologic mucosal improvement, and corticosteroid-free remission were generally well maintained at Week 102 compared to Week 54. The recommended posology for newly diagnosed or newly infliximab-initiated patients, suggests that Remsima® SC should be initiated as a subcutaneous injection at week 0 and additional injections will be given at week 1, 2, 3 and 4 after the first injection. Then, maintenance therapy will be followed every 2 weeks thereafter. The CHMP recommends that regdanvimab can be considered a treatment option for patients at high risk of progressing to severe COVID-19, based on a reasonable likelihood that the medicine may provide clinical benefit, and a low likelihood of harm.1 Under Article 5 (3) of Regulation 726/2004, the CHMP’s scientific opinion provides a harmonised EU- level opinion on the efficacy, quality and safety of antibodies. The scientific opinion can be considered by EU member states when making decisions on the possible use of monoclonal antibody therapies at a national level prior to a marketing authorisation. The EMA has also initiated a rolling review of regdanvimab based on data from animal studies (non- clinical data) and clinical trials, in addition to data on the quality of the medicine. The Part I of the trial enrolled 327 patients with mild-to-moderate symptoms of COVID-19 across three treatment groups (40mg/kg, 80mg/kg and placebo), in which approximately 60% of patients with moderate symptoms suffered from COVID-19 related pneumonia. This data analysis has demonstrated that at Day 28, CT-P59 treated patients presented with a significantly reduced risk of COVID-19 related hospitalisation and oxygenation without mortality. When compared to placebo, CT-P59 (40mg/kg) treated patients reported reduced progression rates to severe COVID-19 by 54% for mild-to-moderate patients and 68% for moderate patients aged 50 years and over. CT-P59 treatment groups also reported with significantly shortened time to clinical recovery ranging from 3.4 to 6.4 days quicker compared to placebo. Patients treated with CT-P59 (40mg/kg) recovered 3.4 days earlier than those in the placebo group [5.4 vs. 8.8 days; 95% C.I, 5.35(3.97, 6.78) to 8.77 (6.72, 11.73), log-rank p-value= 0.0097]. Patients with pneumonia treated with CT-P59 (40mg/kg) reported shortened recovery time by 5.1 days compared to placebo [5.7 vs. 10.8 days; 95% C.I, 5.73 (4.13, 7.33) to 10.81 (6.81, N.C.)]. Moderate patients aged 50 years and over treated with CT-P59 (40mg/kg) reported a shortened recovery time by 6.4 days compared to placebo [6.6 vs. 13.0 days; 95% C.I, 6.64 (4.13, 11.94) to 12.97 (6.81, N.C.)]. In addition, CT-P59 treated patients rapidly and significantly reduced viral load through Day 7 compared to placebo. Top-line results from the trial shows CT-P59 to have a positive safety profile, comparable to that of the placebo group, with no serious adverse advents reported. Infusion related reactions were mild and transient, with the CT-P59 treatment group having reported 0.5%, compared to 1.8% reported with placebo. “The data demonstrate that CT-P59 could contribute to both a reduced time to clinical recovery, as well as a reduced proportion of patients who progress to severe disease.” said Professor Joong-Sik Eom, Infectious Disease Division at Gil Medical Center of Gachon University, “The treatment’s safety profile was comparable to that of placebo and generally well-tolerated.
